Position Summary Under the direct supervision of the Center Manager, the Enrollment Registered Nurse (RN) is responsible for providing pre-enrollment and enrollment nursing assessments, including processing of the DHCS Level of Care form and development of individualized plans of care and goals. Responsible for ensuring efficiency in the enrollment process. Also responsible for developing enrollment reports and auditing RN documentation in the Electronic Health Record (EHR).
Essential Functions of the Job: - Compiles health information necessary to complete Level of Care documentation. Follows all approvals and deferrals of Level of Care documentation and responds to requests for further information from the Department of Health Care Services (DHCS) as needed.
- Performs Comprehensive Enrollment Assessments. These assessments may be conducted at the center, in the patient's home, or at any other appropriate location/facility. Uses standard nursing practices to assess the nursing needs of prospective patients. Identifies and develops specific plans of care and health care goals.
- Presents assessment findings to the Interdisciplinary Team (IDT) on a scheduled basis. Communicates changes in patient status to the IDT. Provides input to other IDT members as needed.
- Participates in nursing care of all enrolled patients.
- Develops reports for documentation of enrollment statistics.
- Audits patient medical records for nursing needs.
- Trains clinical staff on the use of the Electronic Health Record (EHR), both initially and on an ongoing basis as needed.
- Communicates and interacts with patient caregivers and/or family members as needed.
- Provides RN clinical coverage in the center (clinic or day center) as needed.
- In collaboration with the Quality Specialist, conducts regular and ongoing audits of RN notes in the EHR to ensure consistency and appropriate follow-up with participant care plans.
Additional Duties and Responsibilities - Stays current with all regulations and standards to ensure compliance with directives issued by government and regulatory agencies.
- Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
- Attends meetings as required and participates on committees as directed, including but not limited to morning meetings, IDT, clinical team meetings, and quality meetings.
- Performs other duties as assigned.
Job Requirements Education Required: Graduate of an accredited school of nursing. Bilingual (Spanish/English) preferred.
Licensure, Registry or Certification Required: Currently licensed as a Registered Nurse in California. Must present credentials demonstrating ability to meet performance criteria for an RN.
Experience Required: - Has practiced nursing within the last 3 years.
- 3 years of RN experience, with at least 12 months working with frail or elderly populations.
- Bilingual (English/Spanish).
Preferred: - Knowledge and/or experience in senior or elder care settings such as nursing homes, hospice, or adult day health care.
- Familiarity with nursing competencies, care paths, and interactive tools used in skilled nursing settings.
- Ability to triage clinical symptoms and apply care paths and clinical interactive tools prior to communication with physicians.
- Comfortable starting IV lines, administering IV medications, and performing related monitoring.
- Comfortable providing acute care for frail adults, complex wound care, and care for cognitively impaired individuals.
Verbal and Written Skills Required: Excellent oral, written, and interpersonal communication skills.
Technical Knowledge and Skills Required: - Must have the skills necessary to provide care for geriatric patients.
- Must be able to thrive in a team environment.
- Strong organizational and supervisory skills.
- Ability to effectively handle difficult or unusual interpersonal situations.
- Must meet a standardized set of competencies for the position.
Equipment Used: Computer, phones, copy and fax machines.
Working Conditions and Physical Requirements: - Physical clearance for reaching, bending, stooping, crouching, kneeling, and grasping.
- Full range of motion and ability to lift up to 50 lbs.
- Ability to lift and transfer participants.
- TB clearance required.
- Must maintain current First Aid and CPR certification.
Immunizations: Must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Ages of Patients Rendered Care: Adult and Geriatric
Universal Requirements - Pre-employment requirements include I-9 verification, physical exam, background and reference checks, complete application, new hire orientation, and pre-employment TB screening.

OSHA Compliance - Adhere to all OSHA training regulations and required schedules.
PPD Compliance - Required health screenings to be performed annually.
